Your Data, Records, and Privacy
At Llanishen Court Surgery, we are committed to protecting your personal information and ensuring that your data is handled safely, securely and in line with UK data protection law. This page explains how we use your information, who we share it with, and what your rights are.

How we use your information
We keep records about your health and the care you receive. This includes information such as:
- your name, address and contact details
- details of your appointments, consultations and test results
- information from hospitals, community services and other healthcare providers
- relevant information from people who care for you
We use this information to:
- provide you with safe, effective healthcare
- arrange referrals, tests and treatment
- manage appointments, prescriptions and results
- contact you about your care (including SMS reminders)
- support health planning, audit and service improvement
- meet our legal and regulatory responsibilities
We only use the minimum information necessary for each purpose.
Who we share information with
We share information with other organisations involved in your care, such as:
- hospitals and specialist services
- community nursing and mental health teams
- NHS 111 and Out‑of‑Hours services
- pharmacies
- diagnostic services (e.g., radiology, pathology)
We may also share information when required by law, for example:
- safeguarding concerns
- public health notifications
- court orders
- preventing or detecting serious crime
We always share the minimum information needed and only with those who have a legitimate need to see it.
Welsh GP Record
The Welsh GP Record is a summary of key information from your GP record.
It is made available to authorised clinicians in A&E and Out‑of‑Hours services to support your care when the surgery is closed.

Call recording and SMS messages
- Telephone calls to and from the surgery are recorded for training, monitoring and patient safety.
- We may send SMS messages about appointments, test results and health screening.
If you prefer not to receive SMS messages, please let reception know.
Access to your medical records
You have the right to see your medical records, except in exceptional circumstances. There will normally be no charge. Requests should be made to: generaladmin.llanishen@wales.nhs.uk
We may ask for proof of identity before releasing information. In very limited circumstances, we may withhold information if releasing it would cause serious harm or if it contains details about another person.

Sharing information for research and planning
We may share anonymised information for:
- medical research
- clinical audit
- service planning
This information does not identify you and cannot be linked back to you, so you cannot be identified from it.
Your rights
Under UK data protection law, you have the right to:
- access your medical records
- request corrections to inaccurate information
- object to certain types of data sharing
- request that your information is not used for research or planning
- complain to the Information Commissioner’s Office (ICO)
