Practice Area

We can only accept patients who live within our catchment area, as shown on the map opposite. Starting from Gabalfa interchange and going East, our practice area is bounded by the A48M, then Cyncoed Road, Hollybush Road, Glyn Coed Road in Pentwyn, Ty Draw Road in Lisvane and then extends North as far as the crossroads in Rudry. To the West our boundary is defined by the Coryton interchange and the A470.

Registering as a New Patient

If you live within our practice area, you can register as a patient. Please bring the following information to reception:

Registration Forms - To register, you will need to complete two registration forms. You can collect the forms from reception or, to save time, you can download and print the forms at home using the following links: Form 1 (everyone), Form 2 (adults), Form 3 (children).

NHS Number – If you are a UK resident, please ensure that you include your NHS number on the registration form, as the Practice cannot register you without it. You can use the NHS website to find your NHS number.

Repeat Prescription – If you have a repeat prescription from your previous surgery, please drop this into the surgery so we can add your medication to our records. This needs to be dated within the last 3 months.

Temporary Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 16 days. After 16 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
