Some of our services provided are not covered by our contract with the NHS and therefore attract charges. Below are some examples of the non-NHS services we might be able to provide:

Private Paperwork

• Private letters / supporting letters (work, school, housing)
• Bus Pass application
• Dŵr Cymru/Welsh Water reduction application
• Firearm Certificate application
• Holiday cancellation form
• Proof of registration
• Private referral healthcare questionnaire
• Occupational Health questionnaire

Medical Examinations

• Driving medical exam (HGV/LGV/Taxi) 
• Fostering - Adult Health Report 

If you need to see the doctor for non-NHS purposes, you must contact the surgery to arrange this in advance. We only provide private services to people who are also registered with us as NHS patients. Private paperwork can take around 4 weeks on average to be completed. Fees are to be paid in advance with receipt. 

Please note we do not provide: Blue Badge application supporting letters, countersigning of passports/driving licences, fitness to travel letters, fitness letters (extreme sports), gym membership cancellation letters, or PIP supporting letters.
